Membership applications can be submitted on-line below OR by downloading a printable application (located on the right side of the screen) and mailing to:

Payment of membership fees must accompany all mailed applications. Checks payable to MDMHA. On-line application membership fees must be mailed to MDMHA Secretary. Memberships are annual and are payable at the beginning of each calendar year of January 1st. Deadline for membership renewals is February 13, 2010. New memberships accepted anytime during the calendar year.
